Please read the following important information from the school nurse.
7th Grade Updated Physicals are Due December 2!
7th grade is the year that proof of updated immunizations and a recent physical is required by the Department of Health. A physical that was completed during the student’s 6th grade year is acceptable if required immunizations were given. Please obtain a copy and send it to the school clinic. Following are the requirements for students entering 7th grade.
Students must have met the pre-kindergarten and kindergarten immunization requirements, plus:
1 dose of HPV (human papillomavirus) vaccine
1 dose of Meningococcal Conjugate (MCVR) vaccine
1 does of Tdap (tetanus, diphtheria, pertussis) vaccine
